COEUR D'ALENE, Idaho (AP) — Federal authorities have fined a northern Idaho business $14,000 following the death of a Washington state teenager killed while working at a Halloween attraction.

The Spokesman-Review reports in a story on Wednesday that the U.S. Occupational Safety and Health Administration fined the Incredible Corn Maze in Hauser. Eighteen-year-old Jeremy T. McSpadden Jr. of Spokane Valley, Washington, died Oct. 10. McSpadden was a role player in the "Zombie Slayer Paintball Bus" attraction. Witnesses say he was dressed as a zombie when he emerged from his hiding place and ran toward a modified school bus carrying paintball players.

Witnesses say he tripped and fell in front of the rear tires and got run over. Federal officials cited NW Creative Solutions, LLC, which operated the Incredible Corn Maze, for three serious violations.
